Let's Get Started! We understand that beginning this journey can feel overwhelming, but remember, every great journey starts with a single step. We are honored to be part of your journey and look forward to supporting you in achieving your full potential.

Complete the online New Client Intake Form. Fill out the form and click submit. We will receive the form automatically and you will receive a text confirmation of receipt.
Once we receive the online intake form we will call to welcome you to WS. We will also provide information about insurance and getting started.
Let’s do this! We will get you scheduled for your first appointment. If your schedule permits we will also schedule a standing appointment to take the stress out of scheduling each week.
If you are nervous about tech then we have your back. We will set up a pre-session tech check test your tech and feel confident for your first session.
Your therapist will send an email with a link to your meeting at the start of each session. Click the link and your session will begin.

Official ID: Have images of the front and back of your current government issued photo ID available. You will be asked to upload these images to the form.

Insurance Card: Have images of the front and back of your current insurance card available. You will be asked to upload these images to the form.

Information You Will Need: The following information will be needed for the form: The name and contact information of the guarantor for your care, the subscriber for your insurance, and your emergency contact.

TIP: The form is only available online. There is no paper version of this form.

TIP: To avoid potential data loss do not start the form and then leave the tab open to complete at a later time. Do not start the form and refresh your browser or allow your computer or device to go to sleep, restart, or time out. Please ensure your device or computer is updated completely or there may be "glitches" due to device issues.

TIP: This form will take approximately 20-30 minutes to complete. The form cannot be started and saved to complete at a later time. Please ensure you have the time to complete the form once you begin.

TIP: Once you click submit you will receive a confirmation message on your screen and you will receive an email confirmation that contains a copy of your completed form. If you did not receive the confirmation email then your submission was not received.

We recommend new clients begin with once per week sessions. As symptoms decrease most clients opt for every other week appointments for continued care.
We will provide outreaches, follow-ups, and satisfaction surveys to support you. We support you every step of the way.
Clients are encouraged to return if they need a "tune-up".
Self-care and peace of mind need maintenance throughout our lives to achieve our goals.
We are active on social media to cultivate our community and provide a positive presence. We encourage you to participate.
