Rates and Insurance

Dani is an in-network provider for the following insurance companies: Averde, Beech Street, BH Net (Behavioral Health Network), ChoiceCare Network, Cigna, Claremont Behavioral Services, Coventry, Deer Oaks EAP, HealthSmart, Humana, LifeSync,  PHCS, PPO Plus, Principal, Texas Crime Victims, Texas True Choice and others.  Dani is in-network with United Health Care (UHC) and United Behavioral Health (UBH). 

Dani is out of network for Aetna but will file using your out of network benefits. 

She is NOT in-network with Medicaid or MediCare and does not bill for services provided under these plans. If you have Medicaid or MediCare and would like to receive services then it will be based on a private pay agreement.

Please call your insurance company to verify that your policy is active and to verify network status should you have any questions.

If Dani is not an in-network provider on your insurance or if you do not have insurance than private financial arrangements may be available based on need and scheduling.

Fees for outpatient individual therapy are $150.00 for the intake assessment and $100.00 per outpatient individual session.

Contracts with insurance providers may supersede these charges. If your insurance company denies coverage on any claim the balance will be billed to the client.

Clients are strongly encouraged to call their insurance company prior to the first session to determine if Wellness Solutions, LLC or Danielle C. Ellis, MA, MCJ, LPC, NCC is an in-network provider and also to determine what your in-network and out-of-network financial responsibilities are. 

Payment of fees is expected at the time services are rendered. It is the client's responsibility to know and understand their personal financial business. Wellness Solutions, LLC will check a client's benefits and submit this information to insurance as a courtesy. Submission of claims to insurance is not a requirement, as such, clients will be responsible for all fees not covered through insurance for denied claims and incorrect explanation of benefits.
